Creating Score/Option Templates

When creating score-based or option-based questions for a feedback question template, you can determine the the options and/or score weighting for each option.

When creating option-based questions, you will need to provide the options for the questions, for example 0 - unacceptable, 5 - outstanding.

The platform will contain a number of pre-defined templates ready to be used for score-based questions if these are suitable for the question. However, if you wish to add your own option/score template, please follow the guidance below.

  Ensure that you are in the process of adding a scored/option-based question in order to complete the steps below.

Step One

Scroll to the bottom of the Use preset template drop-down menu and click the ADD SCORE/OPTION TEMPLATE button.

Step Two

In the ADD OPTION TEMPLATE pop-up, provide a title for the scoring/option template in the Template Title field.

Step Three

Click the plus button underneath to add your first option. Give a title to this option and provide a description if you wish. This description will appear when the user hovers over the option when they are completing their feedback questions. If you are creating a scored question, you will need to provide a number in the score field.

Then click the ADD button to add this option to the template.

Step Four

Repeat the step above to add further options to the template.

Step Five

When you have selected the scoring for the question, click the APPLY SCORES button to apply this to the question.

  If you do not click the APPLY SCORES button, the scoring will not be saved against the question and a red warning sign will display next to the question in the Feedback Questions details view.
